Levin's Women's Health & Wellness Center in North Miami, Florida welcomes you to our practice. Please complete the patient portal (medical questionnaire) to which an individual link will be emailed to you personally once you have made an appointment. This is not a downloadable, printable form but an intelligent, adaptive online medical questionnaire. Please make sure you complete this questionnaire at the latest the day prior to your appointment as the physicians take their time to prepare and review your medical history the day prior to your appointment. This will ensure that you get the most benefit from your time with the physician.
